Discharge Summary

Helps patients understand their visit and next steps.

The discharge summary page presents when the patient has finished their visit and has received their discharge instructions. It contains information to help the patient understand what happened during their visit, and lists actions they need to take in continuing their recovery outside the hospital.

Share Discharge Summary

Enables the patient to share the contents of the discharge summary with family and friends via an SMS link.

Schedule a Follow-up Appointment

If a patient requires any specialist or primary care follow-up appointments, the following cards will be shown providing details to the patient as to how they can get in touch with their doctors to schedule these appointments.

Pick up your Medications

If the patient has any required medications, this tab shows the pharmacy where they have been sent, alongside the medications they need to pick up.

Understanding your Diagnosis

We show a list of video education prescribed to the patient during their visit. This comes in the following form:

Additional Information

Finally, we show the patient a link to care tasks they may need to complete. These are customizable by the facility and can include elements like:

  • A link to the patient portal.

  • A link out to experience surveys.

  • A link out to an assistance line.

  • A link out to the Billing URL.

  • A link out to general hospital information.

  • A link to contacting hospital support.


Frequently Asked Questions

What are the full note requirements to generate a Care Summary?

  • Must have at least one provider note.

  • Notes must be from the last 24 hours.

  • Must have either:

    • At least 2 notes from enabled sections (Assessment, AssessmentAndPlan, Diet, FollowUp, Goals, LabsOrOrders, Plan, Subjective), OR

    • Up to 2 complete provider notes if there is insufficient section coverage.

What is the full list of requirements that must be met to currently generate a Care Summary?

  • We trigger Care Summary generation at 9am local facility time every day.

  • If this full list of criteria is not met, we continue to check for new relevant provider notes every hour, if at any point this criteria is met, we generate a new Care Summary for the patient.

  • After 10 hours (7pm facility time), if we still have not received sufficient provider notes, we stop trying to generate a Care Summary for that day. We then try again at 9am the following day, repeating the same process listed above.
