How to Manually End a Patient Visit

Mark a visit as ended when a patient is discharged or leaves before the EHR records the outcome. This removes the visit from your active list and keeps your patient census current.

Step 1

Open the visit details and click the End Visit button.

Step 2

A confirmation dialog will appear. Optionally enter a reason, then click End Visit to confirm.

Step 3

The visit will immediately be removed from your active visits list and show a Manually Ended status in the patient list.

Important to Note

Ending a visit removes it from your Care Team view — it does not stop the patient from receiving text messages or using the app. To stop patient communications, click Deactivate Patient Access on the visit details page.

Frequently Asked Questions

The patient left but I don't see a discharge note in the EHR. Can I end the visit?

Yes. This feature is designed for this situation. Mark the visit as ended in Vital while you wait for the EHR to catch up. You can also end the visit when you need to update your census immediately.

I ended a visit but the patient is still getting text messages. Why?

Ending a visit only removes it from your Care Team view. To stop patient communications, click Deactivate Patient Access on the visit details page. These are separate actions.

What if the patient's outcome never shows up in the EHR?

Contact Vital Support with the patient name and visit ID. We can manually set the outcome in the system.

Can I undo ending a visit?

Yes. Contact Vital Support with the patient name and visit ID.

I accidentally deactivated patient access. How do I fix it?

You can reactivate the patient yourself. A red circle icon next to the patient's name in the patient list indicates they're deactivated. Click the Invite icon on the right of the Visit list, or open the visit details and use the Reactivate button, and resend the invite.

Can I see which visits were manually ended?

Yes. The patient list shows a Manually Ended status for visits you've ended.
