Understanding Visit Status

What every value in the Visits list Status column means.

The Status column in the Visits list is the quickest way to see where a patient is in their visit, and whether the visit has ended. Every value comes from the hospital record except one.

The Status column, for active and ended visits

While a visit is open

Active means the patient is currently in their visit. Starting Soon means the visit has a start date in the future, which happens with scheduled admissions that have not yet arrived.

After a visit ends

Once the visit ends, Status displays the outcome recorded in the hospital record.

Status

Meaning

Discharged

Sent home

Admitted

Admitted to a bed. An observation stay also displays as Admitted

Transferred

Moved to another unit or facility

LWOBS

Left without being seen

AMA

Left against medical advice

Deceased

The patient is recorded as deceased

Outcome

The record holds an outcome Care Team has no label for yet

Manually Ended

Manually Ended is the one status Care Team sets itself. It appears when a care team member ends a visit in Care Team before the hospital record catches up, which keeps the census current when a patient has clearly left. The real outcome replaces it once the feed delivers one.

Ending a visit is a Care Team action only. It does not stop the patient receiving messages, and it does not alter the hospital record.

Frequently asked questions

A status looks wrong or has not changed. What does that mean?

Every status except Manually Ended arrives from the hospital feed, so Care Team is reporting what the record currently holds. A status that is stale or plainly incorrect points at the feed or at the mapping behind it, and is worth raising rather than working around.

How is an observation stay recorded?

As Admitted. Care Team does not separate observation from a standard admission in this column.

A patient is no longer here. How are they removed from the list?

Patients are not removed by hand. An ended visit drops off on its own schedule, and a visit can be marked ended early when the patient has left and the record has not caught up.
