How patients log into the Vital app.

When a patient clicks the link in one of our SMS messages, they're taken to the login screen. This screen shows the name of the facility, an overview of the features available once logged in, and a legal disclaimer linking to our Terms & Conditions and Privacy Policy.

To log in, the patient confirms their last name and date of birth. By logging in they agree to our Terms & Conditions and Privacy Policy. If a patient has already logged in during their visit, they won't be prompted to log in again, and if they close the tab or browser, they'll be returned straight to the app without re-confirming their details.

Family members and caregivers logging in for the patient

Patients may need a family member or caregiver to log in for them. To enable this, the family member or caregiver's phone number must be added to the patient's registration record. As long as no one else has already accessed the app for that patient, the family member or caregiver receives an SMS invitation. They log in with the patient's last name and date of birth.

When login fails or the link stops working

Too many incorrect attempts. After ten failed attempts to enter the last name, the visit link in the SMS is deactivated. The clinician can re-invite the patient by direct message or QR code from the patient list in the Care Team app.

Last name misspelled in registration. If the patient's last name was misspelled in the facility registration system and later corrected, they may be unable to log in. The clinician can re-invite the patient by direct message or QR code from the patient list in the Care Team app.

Link shared with other people. Because of the sensitive data shown, only one person can "own" a patient's visit. The first person to log in, patient or not, locks the visit; anyone else using the link sees an error. To share access safely, patients should use the in-app share feature instead of forwarding the link.

How long access lasts

A patient can access the app for 14 days from initial login. Logging in via a new SMS link extends access for another 14 days from that date.

Frequently asked questions

Is my login remembered if I close the tab? Yes. Once a patient has confirmed their last name and date of birth, they won't need to do so again and will be logged in directly.

What if the patient enters their details incorrectly? After ten attempts the SMS visit link is deactivated; the clinician re-invites the patient (see above).

What if the patient's name was misspelled in registration? A corrected last name can block login; the clinician re-invites the patient (see above).
